India’s aviation regulator has identified pilot confusion over the engine-fuel-cutoff switch as a critical factor in the June 12 Air India near-crash. According to the preliminary investigation, pilots inadvertently activated the switch during a high-workload moment, briefly starving the engines of fuel. There were no injuries, but investigators flagged urgent revisions to cockpit design and pilot training protocols. Air India and the Directorate General of Civil Aviation have pledged to implement corrective measures swiftly to prevent similar incidents.
